After extensive experience as a student clinician in an inpatient rehabilitation setting, I created a collection of student resources to help occupational therapy students manage a caseload while on fieldwork.

The following resources were created for use in an inpatient rehabilitation hospital and can be modified to use within a different practice setting.

Daily Planning Template

The Daily Planning Template was designed to help students organize their daily treatment schedule by providing space to document pertinent patient information, patient goals, and intervention ideas.

Goals and Intervention Sheet

The Patient Goals and Intervention Sheet serves to help student clinicians develop tailored, effective interventions given each patients' therapeutic goals. This sheet was designed to improve clinical critical thinking skills by encouraging students to carefully consider client factors and barriers to participation when designing an intervention plan.

Patient Information Sheet

The Patient Information Sheet was created to help student clinicians identify and keep track of patient information including discharge date, key goal areas, and anticipated medical equipment for home.

Plan of Care Template

The Plan of Care Template was developed to help students visualize and prepare a plan of care given each patients' anticipated length of stay. The sheet promotes understanding of the skills a patient must acquire each week to meet goals in preparation for discharge.

The Progress Note Tracker was designed to help students track progress notes and patient conferences as well as to improve interdisciplinary collaboration and communication.
